 After clicking, you will be redirected to google form on the Student Union CTU disk. It has a pre-filled first section, please do not interfere. In the event that some information does not fit, please contact [[mailto: kancelar@sh.cvut.cz | club office]].

 If you would like to share this process with your friends, share with them link to this page, not google form, because pre-filling to this form is unique only for you and it is done by clicking on the button in your page on information system. So if they would like to create their request, they need to visit their own profile.
